Do I have to wait until my child has a physical before I can register them?
No. You can register now and upload the physical exam form later. The physical exam form is due 14 days before the start of your camp session.
I have a physical exam form for my child for a different camp or a sports team. Will you accept that?
Yes, provided it has essentially the same information and is signed by a licensed physician. It also must be dated within the last 12 months.
Why do I have to provide an immunization record for my camper?
To protect all of our campers, we strongly enforce our immunization policy. Our policy follows Minnesota state law regarding immunization schedules. We require campers to be fully vaccinated and up-to-date for the current calendar year on their Covid-19 vaccinations. Camp Quest North does not allow conscientious objection.
Campers who cannot receive immunizations due to a medical reason will be allowed to attend camp if they provide a medical exemption signed by a licensed physician.
More information regarding immunizations can be found here: http://www.health.state.mn.us/immunize
